Each senior enrolled in PACE has a specific care team dedicated to their ongoing needs. This team includes the senior’s primary care providers, nurses, dietician, home health care providers, dentist, rehab therapists, activity coordinator, day center manager, and transportation providers. All of these individuals are devoted to providing personalized and compassionate care to your loved one on a daily basis to promote his or her overall well-being. Not only will you have peace of mind that your loved one’s needs are met but there will also be no lingering worries about how you will pay for their care. PACE accepts both private pay and Medicaid.
