One of our flagship offerings is McGregor PACE, [the Program of All-inclusive Care for the Elderly] considered the Gold Standard Model of Care designed to provide the entire continuum of care and services to older adults while being able to maintain their independence in their own home and community for as long as possible.
This community-based program provides services through an interdisciplinary team (IDT) and a health plan that focuses on comprehensive care with full integration of services while having access to Adult Day Health Center that addresses the medical, rehabilitative, social and personal care needs wherever they call “home”.
