McGregor PACE Hosts Webinar for the Community

Understanding the PACE Model of Care for Seniors

Many seniors wish to age in place, but are unable to receive the same level of care in their home than they would in a nursing home environment. The McGregor PACE program offers comprehensive, coordinated care for seniors and their families to allow them to safely remain in their homes within the community. On May 28, 2021, McGregor PACE hosted a webinar featuring several team members in a discussion about what sets PACE apart from other senior care providers.

The team discussed several primary reasons why the PACE model of care is unique in Cuyahoga County and how it is a significant benefit to eligible seniors and their families:

  • PACE uses a managed care model. There is a team of professionals that work with PACE participants and their family members to decide the best approach for their care—that is why we refer to them as participants, not patients. It is a community-based approach to receiving long-term care services.
  • PACE is team based—and that is unique in community care. While you do find team based care within hospital systems, it is a very different approach from what you would normally find in outpatient community care and primary care settings. Our program involves a core team that includes a physician, advanced practice nurse, registered nurse, and a licensed social worker supplemented by additional health care professionals.
  • PACE supports the caregiver. As Dr. DeGolia said, “If we can support the caregiver to enable them to continue to care for their loved one in the community, that’s half the battle, and we work hard to do that.” The McGregor PACE team knows that social or environmental situations can impact the health condition of a senior. By supporting the caregiver, we can in many cases reduce stress.
  • PACE offers rapid access to care. Many seniors make countless trips to the emergency department for care before enrolling in PACE. When they enter the PACE program, those visits drop substantially because of increased access to care and responsiveness to their health needs.
  • PACE provides respite care for families. For example, sometimes caregivers have a vacation planned, are dealing with a personal illness, or just need a break—and PACE pays for a respite stay for the senior so that the family can take that much-needed break and take care of their personal needs.
