How Alfonzer Brooks Found a Caring Community at PACE
Personalized Therapy to Promote Independence
One in four veterans in the United States has been impacted by a service-connected disability. Alfonzer Brooks is one of those veterans. When Alfonzer found PACE, he was facing spending the rest of his life in a nursing home. PACE helped him find alternative housing, took over management of his medications that he was receiving as part of his benefits through the Department of Veterans Affairs, and gave him an opportunity to live in the community.

A retired veteran and a widower, Alfonzer first met with his PACE social worker, Emily Cutler-Brockway, in March of 2017. Unlike if he had stayed in a nursing home, PACE provided him with an individualized therapy program. He has built a relationship and rapport with his occupational and physical therapists and works toward his therapy goals every single day. Along with focusing on his therapy, Alfonzer has made many friends, participates in weekly Bible studies, and gets to participate in group outings.
Emily saw a great change in Alfonzer following his enrollment in PACE. She noted, “There’s a brightness to him [now]. He’s not isolated, he has a lot of friends…he participates in therapy every day.”
Alfonzer has fully committed to his rehabilitation at PACE. He uses his care team and the resources available to him to improve his lifestyle and maintain his independence. As a result, he is able to benefit from all of the opportunities that PACE provides.
“You came here to work, and we came here to make sure that you’re going to get the best help that you possibly can. And that’s what I love about being here,” said Alfonzer.
