How to Enroll in McGregor PACE in 2021

Begin the Enrollment Process Today

If you have been considering enrolling in the PACE program in 2021, now is the time to take the first step. PACE is an ideal solution for many Cleveland seniors who want to continue to age in place but also need additional support and care. The need for PACE has been heightened for many seniors during the pandemic, with opportunities for safe socialization and activities becoming even more critical. 

Have you been waiting to enroll due to COVID-19? Let us assure you that enrollment is ongoing. Our centers remain a safe destination for seniors, we are following all COVID-19 related protocols, and we continue to enroll new PACE participants every day. The enrollment process is straightforward, and our team will be there every step of the way! We want to provide a bit more explanation about what you can expect during the PACE enrollment process.

Starting the Intake Process

Your first step is to call our office at 216.791.3580. Our team will provide an overview of how PACE works as well as the specific eligibility requirements. To help you determine your eligibility, we will ask you a series of questions during this phone call. Before we end the call, we will help you schedule an at-home assessment as well as a virtual tour.

Meeting With a McGregor PACE Team Member

A big focus during the enrollment process is determining your or your loved one’s unique needs. For example, you may need physical therapy as part of your care plan, or you may be interested in our transportation services to and from our day centers. You will meet with a member of our interdisciplinary team—usually a nurse, social worker, or occupational therapist—and then that individual’s assessment will be reviewed by our Medical Director and the entire interdisciplinary team at McGregor. 

Determining Your Eligibility

Our team will then start the application process to determine your or your loved one’s medical and financial eligibility for PACE. We will make sure you understand all of the documents required to complete the application forms—ranging from proof of health care insurance and assets to proof of citizenship—and help you through that process.

Completing Enrollment

Once you or your loved one has been approved for enrollment into PACE, we will schedule a meeting to finalize plans for beginning PACE. This meeting usually includes the PACE team, the participant, and the participant’s family members or primary caregivers. Once enrollment in PACE is complete, you or your loved one can begin to enjoy and benefit from our many services.
